While the doctor was right about the small portions, he does not need to be eating whatever he likes. He still needs to be eating a well balanced meal. Fried food is a definate no no, although he can occasionally indulge just like any person.
There will be some foods that your friend just won’t be able to eat because it’ll seem to "go right through" him. He’ll have to learn those on his own. My mother was the same way. She can’t eat ice cream anymore because of her surgery. It doesn’t settle well on her stomach but she LOVED it before the surgery. References :
I had gastric bypass about 8 years ago and I can eat whatever I want. However, I try NOT to eat a lot of junk and fast food is an occasional thing. I try to eat right. What the doctor means is that there are no longer any restrictions on foods, and by now your friend should know what he can and cannot tolerate. (For me, its sometimes rice and pasta.) I’m sure the doctor didn’t say it’s okay to eat fast food for every meal – he isn’t responsible for what your friend puts in his mouth. I will say this, that NO ONE could ever live the rest of their life after gastric bypass on any sort of "prescribed diet" as those have been proven to fail, time and time again. Any diet that restricts foods or forces you to eat certain foods will, long term, fail. Most people can do it for a few weeks at best, and there are rare individuals who can do it, but most people can’t. That’s why most fad diets fail.
You can berate your friend all you want about what he is eating, but in the end, he’s the one that will pay if he is making poor choices – weight gain and high cholesterol to name a couple. References :

Gastric bypass is supposed to be a tool towards getting a lifestyle change to eat healthy and change those old eating habits that caused so many problems and contributed so much to being obese. Even though your friend cannot eat as much, he still can take in quite a lot of extra calories with the choices he is making.
